About the patient
◾ 35 year old male, no relevant chronic non-surgical history
◾ Box of lumbar pain of more than 6 months increasing in intensity and the last weeks with dysesthesias in dermatome L5/S1
◾ It was managed conservatively with analgesics and physiotherapy without a positive response to it

Surgical treatment
The surgical treatment was decided considering age and MRI data: ◾ fusion of the segment through a posterior approach with transpedicular screws
◾ use of DIPLOMAT system due to its versatility, efficient grip of the screws and good handling of the instruments
